Будучи “считанным отказавший после 0 из 4 096 в xxxxxx: ошибочная ошибка” ввода/вывода для LVM

У меня есть система с Ubuntu 12.04. В этой системе используемый LVM для управления дисководами и подобными устройствами массового хранения. Когда я использовал lvscan команда для сканирования всех логических томов затем это отобразило следующий вывод
